In this rewarding role, you’ll provide support that empowers clients to maintain their independence and remain actively involved in their care. This community-based role involves travelling between customer’s homes and delivering personalised, day-to-day support aligned with individual care plans.
About You:
- Certificate III in Aged Care, Home & Community Care, Disability, or equivalent
- Ability to understand and follow individual care plans
- Strong communication skills with clients, families, and caregivers
- Relevant experience in home or community care settings
- Current First Aid Certificate
- Current Victorian Driver’s Licence
- Evidence of current flu vaccination
- Valid Working with Children Check or willingness to obtain
- Current Police Check
- NDIS Worker Screening Check or willingness to obtain
