Our qualified Registered Nurses provide professional care in the comfort of your home. We’re here to support you and your family with a range of services, including:
- Shower assessments.
- Wound care and dressings.
- Blood tests and pathology collections.
- Support for carers.
- Monitoring blood pressure, vital signs, and overall health.
- Palliative care and bereavement support.
- Support for chronic conditions such as diabetes or respiratory problems.
- Help with taking your medications safely.
- Liaison with your GP and coordination of referrals to other services.
We also support people who have recently returned home from hospital or are receiving hospital-in-the-home care.
Community Nursing services can be provided in your own home or at one of our campuses in Warracknabeal, Hopetoun or Beulah. We service the Rural Northwest catchment.
Operating Monday to Sunday (except public holidays).
Accessing a Community Health Nurse
A referral is required through My Aged Care for clients 65 years and over.
Clients under 65 years do not need a referral.
