Update for our communities on COVID19 - RAT self-reporting

RNH would like to inform you that the self-reporting positive rapid antigen test (RAT) form is unavailable online on the Victorian Coronavirus hotline. We will communicate further information about positive RAT self-reporting in due course.

In the meantime, please follow the below advice.

Managing COVID-19 at home | Coronavirus Victoria

  • For worsening symptoms, contact a GP or GP Respiratory Clinic. You can contact the National Coronavirus Helpline on 1800 020 080 (24 hours a day, 7 days a week). If you can’t see a GP, you can contact the Victorian Virtual Emergency Department for urgent care.
  • Please note that the COVID Positive Pathways Program has ceased, with final patients enrolled on 1 June 2023. If someone eligible has submitted their positive rapid antigen test result on or before 1 June, they will continue to receive care from the COVID Positive Pathways program.

With a large range of services, Community Health aims to be responsive to the needs of the community while working together with individuals and families to provide a person-centred approach. Our services are affordable and local to ensure that all members of the community can access and receive quality health services.

Community Nursing

Our Community Nurse Service provides trained nurses to support people in their home throughout the Rural Northwest Health catchment. Working closely with each client as well as their carers, GPs and community health clinicians Community Nurses provide coordinated care to the home. Services include in-home nursing and treatment, administering and supervision of medication, wound care, post-acute care, specimen collection, palliative care and Advance Care Planning.

Wellbeing Team

Wellbeing coordinators will help you to live well. They are your first point of contact when you need help. Our wellbeing coordinators will work with you to arrange care and support, no matter what your age or need. They will help you with support for family, friends and carers.

Medical Imaging

We have the only 3D/4D Ultrasound machine in the Wimmera Mallee area. With 3D ultrasound, a series of thin 2D slices is digitally reconstructed to give more life-like images. With 4D ultrasound, the added dimension is time, so that the 3D images appear to be moving in real time. All X-Ray requests are also available at our Warracknabeal campus.

Home Volunteer


The volunteer program has approximately 55 active volunteers. Roles that the volunteers undertake are wide and varied with some volunteers offering 1-2 hours each month and others offering several hours or days weekly. Each and every volunteer is valued greatly as the time that they give enhances the lives and social outcomes for the residents.
