We are seeking a motivated and compassionate Intake Clinician to join our Community Services Intake Team. As the first point of contact for people accessing community health services at Rural Northwest Health, you will play an important role in ensuring timely access to the support they need. Using a responsive, person-centred approach, you will coordinate referrals, assess service requirements, and guide individuals through available
health and community care options. To be successful in this role, you will have:
- Qualifications as an Allied Health Assistant, Registered Nurse, or Allied Health Professional.
- Experience delivering person-centred care within a community health setting.
- Excellent written, verbal, and computer skills.
- Strong time management and organisational skills, with the ability to prioritise competing demands.
- Highly developed interpersonal skills and the ability to work effectively within a multidisciplinary team.
- The ability to manage your workload independently while contributing positively to team goals.
- Exceptional attention to detail and administrative accuracy.
- A commitment to ongoing learning, professional development, and continuous improvement.
- Knowledge of primary health care and chronic disease management principles (desirable).
- Current influenza vaccination evidence.
- A current Working with Children Check and Police Check (or willingness to obtain).
- A current driver’s licence.
