Our District Nurse Service provides trained nurses to support people in their home throughout the Rural Northwest Health web.
District Nurses work closely with each client as well as their carers, GPs and community health clinicians to provide coordinated care to the home.
District Nursing services include in-home nursing and treatment, administering and supervision of medication, wound care, post-acute care, specimen collection, palliative care and Advance Care Planning.
Referrals for this service can be made by GPs, community groups and hospitals but self-referrals or from family members are also accepted with approval.
Fees may apply but not always. They are set by the Department of Health and are based on each individual’s financial situation. We will discuss this at your first visit.
