Wimmera Palliative Care Service supports people across the region to receive individually tailored palliative and end-of-life care in their own homes.
The Wimmera Palliative Care team works closely with clients’ General Practitioners, local services, and Rural Northwest Health Community Services to help deliver holistic care close to home.
Collectively, we support comfort, dignity, independence, and quality of life for people living with life-limiting illnesses while also providing guidance, reassurance, and support to families and carers throughout their journey. Together, the Wimmera Palliative Care team, General Practitioners, District Nurses, Allied Health team, and Home Support team provide local home-based nursing, allied health, and supportive care tailored to the needs of clients and their families.
With this week marking National Palliative Care Week, we would like to thank all staff from the Wimmera Palliative Care Service, General Practitioners, Rural Northwest Health Community Services, and other health and support services for the compassionate care and support they provide to clients and families throughout our community.
