With campuses in Warracknabeal, Hopetoun and Beulah, and conveniently located between Adelaide and Melbourne, Rural Northwest Health provides a wide range of services including acute, urgent care, residential aged care, and comprehensive community health and allied services.
We are seeking an Intake Clinician to join our Intake Team, providing intake support to consumers accessing Community Services across all Rural Northwest Health campuses, working 64 hours per fortnight. In this role, you will deliver a responsive and person-centred intake service, coordinate referrals and connecting consumers with the most appropriate services. This is an excellent opportunity to make a meaningful difference by improving access to quality healthcare for rural communities.
The successful candidate will have:
- Qualifications as an Allied Health Assistant, Nurse or Allied Health Professional
- Experience in the implementation of person-directed care within a community setting
- Excellent written and verbal communication skills including computer skills
- Strong time management skills with the ability to prioritise tasks
- Strong interpersonal skills and demonstrated collaboration within an inter-disciplinary team
- Ability to plan and organise own workload to support the functions of the team
- Excellent attention to detail
- Commitment and willingness to participate in continued training and education related to the role
- Knowledge of Primary Care Principles and chronic disease management principles is advantageous
- Current flu vaccination evidence
- Current Working with Children Check and Police Clearance or willingness to obtain one
- Current driver’s licence
