The Home and Community Care Program for Younger People (HACC PYP) supports those who need assistance with activities of daily living- Delivered across the Yarriambiack shire.
Optimising your health and wellbeing, while also assisting you to maintain, or regain, your independence to live safely at home and actively participate in the community, the program targets people aged under 65, and Aboriginal and Torres Strait Islander people under 50.

Fees and Charges
A Living at Home Assessment is free. However, the cost of any services that may benefit you will be discussed during your assessment.
