Our Wellbeing Coordinators are a team of health care workers who are available for consumers and their families seeking support navigating the health care system.
Wellbeing coordinators provide assistance with My Aged Care, Advance Care Planning, Cardiac Monitoring, Services Australia, Telehealth support and welfare checks.
